East Gippsland Primary Care Partnership

The East Gippsland PCP is a voluntary alliance of health and community agencies working together to enhance the wellbeing of all the people in the East Gippsland catchment. 

The PCP provides a framework to improve the planning and delivery of primary care services, as well as, ensuring services work effectively together to place consumers at the centre of service delivery.

PCP priorities are:  Integrated Health Promotion, Service Coordination and Integrated Chronic Disease Management through a partnership approachThe East Gippsland PCP Strategic Priorities for 2010-2012 are:-
