About Us

The East Gippsland Primary Care Partnership (EGPCP) is one of 28 Primary Care Partnerships across Victoria. The EGPCP is a voluntary partnership of health and community support agencies working together towards achieving better health outcomes for the community across the East Gippsland local government area. EGPCP play an important role in facilitating and sustaining effective partnerships, supporting collaborative change and shared goals in East Gippsland.

Our Strategic Plan

The Strategic Plan 2018-2022 supports the implementation of the East Gippsland Health and Wellbeing Plan ‘Well Placed for Wellbeing’ and is aligned to the Victorian Public Health and Wellbeing Outcomes Framework. It guides the work of the EGPCP, outlining the priority areas and key deliverables.

Our aims:

  • Facilitate and coordinate outcomes-focused, integrated prevention planning on changes outline in Well Place for Wellbeing for: Being active and eating well; Protecting our health; Resilient and connected communities.
  • Enhance access to health services through interventions supporting organisational health literacy and cross-agency coordinator of services.
  • Support existing partnerships and networks to identify gaps and strengthen local prevention of family violence initiatives in line with Well Placed for Wellbeing.
  • Develop and sustain strategic partnerships and communications that support health and wellbeing in East Gippsland.
